Cardiology medical billing services in Montana

Montana cardiology practices are dealing with several Medicaid changes at once. Passport to Health has given way to Primary Care Montana, the provider portal now sits behind a single sign-on login, and a new claims system is coming. Cardiology Billing Services keeps claims accurate through those changes and through the usual coding demands of heart care.

We support the full cycle for Montana groups: eligibility, authorization, coding, claims, payment posting, denials, A/R and enrollment.

How Montana Medicaid shapes cardiology billing

Montana Healthcare Programs are offered through Montana Medicaid by DPHHS. Benefits, authorization, claim routing and appeals are dictated by the program the patient is in. The first thing to do is to determine what program the patient is actually enrolled in.

Two patients with Medicaid cards may take separate paths. A child enrolled in Healthy Montana Kids (HMK) is billed through Blue Cross and Blue Shield of Montana. A child enrolled in HMK Plus is billed through Medicaid. An older adult may have Medicare as primary and coverage can vary from the time of the visit to the time the claim is made.

Eligibility Check Coverage Enroll Provider Referral and Authorization Claim Payment Follow-up

Medicaid fee-for-service rules

Montana operates Medicaid predominantly fee-for-service with a primary care case management model layered on top. Claims are submitted via a provider portal and a state fiscal agent. There is no managed care plan roster to go through.

  • Verify eligibility on the date of service.
  • Confirm the service is covered and medically necessary.
  • Look up prior authorization rules for that service.
  • Document the clinical reason and send a clean claim.
  • Track status and remittance, then correct or appeal denials.
Portals and systems

Montana Healthcare Programs portal and system changes

Provider Services Portal – Eligibility, Claims Submission & Status, and Provider Record Changes. Provider Relations has a phone line for these kinds of questions. MATH (Montana Access to Health) is another web portal that offers eligibility and claim lookups.

01

ICAP single sign-on

Access to the Provider Services Portal now runs through ICAP. Every user logs into their ICAP account, opens the Provider tile and goes to the portal. Practices should ensure all billers and clinicians who need access have registered.

02

Montana Health Care Claims Database

DPHHS has contracted with Gainwell Technologies to replace the MTHCS claims system. There may be new trading partner requirements for billing vendors and clearinghouses.

Until the new system is live, keep using the current submission process. Portal screens and EDI details can change, so old instructions are worth rechecking.

Provider records

Medicaid enrollment records for cardiology groups

Individual and group records need to match. A mismatch in identity, taxonomy, location or billing relationship can hold up a claim before anyone looks at the coding.

Enrollment details to review

  • Individual and group enrollment records.
  • NPI, taxonomy, specialty and license.
  • Billing and rendering provider relationships.
  • Service locations, ownership disclosures and provider agreements.
  • Revalidation records and routine updates.

Ordering and referring providers

All providers who order, refer or prescribe for Montana Healthcare Programs members must be enrolled, even if they do not bill the program. (42 CFR 455.410) Cardiologists should follow up on tests or referrals that they order.

Previous affiliations with MTHCS

DPHHS has informed providers that once MTHCS is implemented, any claim received by clearinghouse, paper or the portal that does not have an affiliation between the rendering and billing provider will be rejected by the portal. Providers can now affiliate and groups should not delay.

Enrollment and credentialing are separate: Enrollment with the state is separate from commercial credentialing. Montana Medicaid enrollment does not put a practice into any commercial network.

Referral coordination

Primary Care Montana and cardiology referrals

Primary Care Montana (PCMT) is the state’s new primary care model replacing Passport to Health, Comprehensive Primary Care Plus and the patient-centered medical home program.

Once the transition is complete, claims for services will no longer need to include Passport referral numbers. Billing teams should delete old referral edits and review denial history so that a retired Passport rule does not hold claims. A referring practice or health system can still initiate a referral on their terms.

PCMT is a voluntary program for primary care and therefore does not affect the payment for each cardiology service. Ongoing communication with referring primary care offices is an important component of follow-up and care of heart failure patients posthospitalization.

Pediatric cardiology

Healthy Montana Kids and pediatric cardiology billing

Congenital heart evaluations and pediatric cardiology visits depend on getting the child's coverage right. HMK is the CHIP coverage group. HMK Plus is children's Medicaid. The two are not interchangeable.

01

HMK

Most HMK medical claims and prior authorizations go to Blue Cross and Blue Shield of Montana, which administers the coverage. HMK has its own provider network and is not subject to the Medicaid EPSDT rules.

02

HMK Plus

HMK Plus follows Medicaid coverage and billing rules, with its own authorization process and network. A child's enrollment shows as Medicaid for HMK Plus and as HMK/CHIP for HMK.

When a child's coverage changes: Children can move from one pathway to the other. State guidance for a hospital stay that spans both has the provider bill BCBSMT first for the days of HMK eligibility, then send the claim with the EOB to the Medicaid fiscal agent. We check the child's program for each date of service.

Pre-service financial clearance

Montana requires prior authorization for cardiology services

Requirements vary by payer, product and CPT or HCPCS code, network status and place of service. Many cardiology services require no authorization at all. This requirement is validated against the member's actual plan and the payer's existing policy.

01

Echo and stress testing

Coverage, coding, medical necessity and authorization guidelines.

02

Cardiac imaging

Nuclear imaging, myocardial perfusion, cardiac CT, coronary CTA, and cardiac MRI.

03

Procedures

Diagnostic catheterization, electrophysiology and interventional services.

04

Authorization-to-claim match

Approved dates, servicing provider, location and codes against the submitted claim.

Eligibility Plan Procedure review Authorization Documentation Tracking and claim match

Important: An authorization does not guarantee payment. The claim still has to meet the payer's coverage, coding, documentation and filing rules.

Commercial and government plans

Blue Cross and Blue Shield of Montana cardiology billing

BCBSMT is a division of Health Care Service Corporation. It sells several products and administers HMK claims. Network status, authorization rules and medical policies can differ by product, so the plan is identified before advanced imaging or a procedure is scheduled.

BlueCard members from other states may follow their home plan's rules. We confirm those through the BCBSMT provider portal.

Other commercial cardiology billing in Montana

Commercial rules stay separate from Medicaid rules. Each plan sets its own network, referral, authorization, medical policy, submission and appeal requirements. We verify benefits member by member and work from current payer policy, not a stored code list.

Imaging and diagnostics authorization

We verify eligibility, insurance product, CPT or HCPCS code, medical policy, supporting documentation, authorization status and dates of approval, servicing provider and place of service before service. We pull their authorization lists again – we don’t trust an old copy because payers revise their lists.

Medicare and coordination of benefits

Medicare cardiology billing in Montana

Montana is in Medicare A/B MAC Jurisdiction F and is served by Noridian Healthcare Solutions. Noridian helps with claim status, Local Coverage Determinations, requests for medical review, reopenings and appeals.

Cardiology claim review by location

E/M office visit billed same day as diagnostic test, ECG interpretation, echo, stress and nuclear investigations, rhythm monitoring, catheterization and treatments. The applicable LCD must be supported by diagnosis codes and medical necessity documentation.

Billed for Medicare Advantage and Dual Eligible

Medicare Advantage plans can vary from Original Medicare on networks, referrals, authorization and appeals. The plan is known before the service and it has got its own rules.

Medicare and Montana Medicaid coordination

For patients covered by both Medicare and Montana Medicaid, we explain which is primary, how crossover works and what the patient is responsible for.

DPHHS requires a “Not Enrolled with Medicare” attestation form with each Medicaid claim for a dual eligible member if the billing provider is not enrolled in Medicare. It is good for 365 days from the date signed and the date of service must be within that time frame.

Cardiology-specific coding

Cardiology coding and diagnostic billing

Our coding review encompasses CPT, HCPCS, ICD-10-CM, modifiers, NCCI edits, bundling, units, place of service and documentation so the claim reflects what is supported by the record.

Office E/M, ECG, echo and stress testing

Code selection, documentation, diagnosis support and edits.

Monitoring and imaging

Holter and event monitors, remote monitoring where it applies, cardiac CT and MRI.

EP and device services

Electrophysiology, pacemaker and ICD services and related procedures.

Catheterization and advanced procedures

Cath, PCI, structural heart, heart failure care and cardiac rehab.

Professional and technical components

Who owns the equipment, who performs the test, who reads and bills for it all determine how a diagnostic is billed (globally or split between professional (modifier 26) and technical (TC)). Some codes are valid for modifier 26 and TC only. Each hospital-based cardiologist, office practice and shared imaging arrangement needs to be reviewed separately.

Operational risk

Revenue cycle issues Montana cardiology practices should watch

Verification of coverage

HMK, HMK Plus and Medicaid come through separate ways. If you guess wrong, the claim goes to the wrong place.

Sign up as a provider

NPI, taxonomy, location, group affiliation and ORP information should be current and consistent.

Pre-authorization

Rules change by payer, product, procedure and place of service.

Charging for diagnostics

Billing for a test depends on setting, ownership of equipment, who reads the study and place of service.

Medical need and coding

Cardiology claims rely on the note, the code, the modifier and the payer’s policy all being in sync.

Coordination of benefits

Medicare, Medicaid, Medicare Advantage and commercial coverage might overlap for the same patient.

Referrals from rural areas and multiple venues

Patients are seen by physicians at multiple sites or move from town to town, which complicates professional and facility claims.

Aging accounts receivable

The filing and appeal windows are still open and the outstanding balances are apparent with regular follow-up.

Remote assistance

Supporting cardiology practices throughout Montana

Montana is the 4th largest state by size, hence many people have to travel to visit a cardiologist. The main hospital-affiliated cardiology groups and practices are in Billings, Missoula, Great Falls, Bozeman and Helena. Smaller towns like Livingston and Lewistown might use outreach clinics or referrals, but Kalispell, Butte, Havre and Miles City depend on regional hospitals and referral networks.

These partnerships result in professional and facility claims, multiple sites of service and outside region referrals. As we go through the cycle, it’s our job to keep eligibility, provider, location, payer, documentation and claim details in line.

Area of service: We provide remote support for Montana practices. This page does not display a Montana office, local staff or in-state facility.

Training procedures

Why Montana cardiology practices outsource their medical billing

Practices hire billing staff to stay up with administrative burden, tighten up eligibility checks, keep up with authorizations and payer changes, and keep a closer eye on denials and aging A/R.

Practice payer mix, documentation and current processes all have an impact on results. We concentrate on regular procedures and clear follow-up and do not warrant any specific reimbursement or collection result.

Frequently asked questions

Frequently asked questions concerning cardiology billing in Montana

Are all cardiac procedures requiring pre-authorization?

No. It is contingent upon the payer, product, service, code, network, and site of service. We examine the member’s current plan before the service.

What’s the story with Passport to Health?

Primary Care Montana was introduced alongside CPC+ and the medical home program. Claims no longer require Passport referral numbers. Please examine existing referral edits.

How does HMK affect billing in pediatric cardiology?

HMK and HMK Plus are handled separately, on different networks, with different authorizations and claim routing. Most HMK claims and prior authorizations are processed by BCBSMT. HMK Plus is billed as Medicaid. We look at the child's program on each date of service.

Are you processing Medicare Advantage cardiology claims?

Yes. We separate the plan and manage its network, referral, authorization, claim and appeal rules from Original Medicare.

Are you able to support certification and revalidation?

Yes. This includes Montana Medicaid, Medicare, commercial payers, CAQH, revalidation, record keeping and group memberships.
