Split billing
One echo can generate a technical claim and a professional claim. We confirm who owns the equipment and who reads the study before either claim goes out.
Cardiology Billing in Iowa
Cardialogy Billing manages the full claim cycle for Iowa cardiology groups, from the first coverage check to the last aging balance. We deal with the three IA Health Link plans, Wellmark, Medicare through WPS and Medicare Advantage, and we create each claim to the rules of the payer that will ultimately pay the claim.
Why cardiology is different
A single echo can produce two claims.A doctor's appointment, stress test on the same day cancel each other out on paper.Device checks, imaging approvals, and edit pairs add more ways for a payment to stall. We build checks around each of these before a claim is released.
One echo can generate a technical claim and a professional claim. We confirm who owns the equipment and who reads the study before either claim goes out.
A visit and stress test on one date of service require modifier 25 logic and clear diagnosis linkage else the visit is packed away.
Many payers need pre-authorization for nuclear investigations, cardiac CT and certain MRI . We match the claim to the permission number, CPT code and location of service.
Pacemaker and ICD checks have various codes for in-office vs remote tests with different frequency restrictions.
NCCI edits in cath, PCI, and EP cases decide whether a second code is paid. We read the procedure note before choosing modifier 59 or an X modifier.
Many Iowa cardiologists have satellite clinics in minor hospitals remote from their main practice. We set the rendering location and place of service for each site.
Iowa Medicaid
The Iowa HHS runs Medicaid, although the majority of members get their benefits via one of three managed care organizations: Iowa Total Care, Molina Healthcare of Iowa, or Wellpoint Iowa. Each has its own provider portal, authorization list and payer ID.
Hawki and the Iowa Health and Wellness Plan operate under the same managed care contracts. We read each member record separately because a child and an adult could be on the same plan but have different benefit packages.
Some members stay outside managed care. Their claims go to Iowa Medicaid directly, so we check for this before assuming a plan and sending a claim to the wrong place.
Commercial coverage
The Blue plan for Iowa is Wellmark Blue Cross and Blue Shield, however cardiac groups also see patients from other states.Omaha, Council Bluffs, Quad Cities, Sioux City, Dubuque, all on a state line, so you get a lot of out of state cards. A patient from Nebraska or Illinois may also be on Medicaid from that state, and that typically implies the practice needs to be enrolled there as well.
Medicare and dual coverage
Original Medicare claims in Iowa go to WPS, the Medicare Administrative Contractor for Jurisdiction 5.The card in the patient’s hand is the tip of the iceberg. Medicare Advantage and Medicare-Medicaid dual coverage have separate restrictions.
WPS local coverage policies apply to claims, including those for cardiac CT angiography and noninvasive FFR. We confirm diagnostic support is in place before testing to prevent a gap to be detected post denial.
Each plan sets its own approvals, network, and appeal route. We confirm the plan name on the card at every visit, because many of these cards look like standard Medicare.
Medicare is the primary payer and Medicaid or the member's managed care plan pays the cost share. We check how the crossover comes to the plan, and follow up on any balance that has not rolled over.
What we handle
All the steps in the revenue cycle are in one process. So administrators know where claims come in, where they get hung up and who is working on each one .
The Medicaid plan, Wellmark product, Medicare type, secondary coverage and deductible status are all verified before the patient visit.
Payer rules are followed, supporting documents are collected, filed and tracked. Each approval is directly linked to the claim it supports.
CPT, HCPCS, ICD-10-CM, modifiers and units are checked against the signed chart note to ensure accuracy.
Charges are pulled from cath logs echo reports and device clinic notes to make sure no billable service is missed or left unentered.
NPI, taxonomy, place of service and unit counts are reviewed by software and a human reviewer before the claim is published.
Electronic claims are sent to the payer ID and every acknowledgment is recorded until the claim is officially accepted.
ERA and EOB data are matched to the claims and any differences are compared to the contracted rates.
Each denial is labeled with a reason code, corrected, resubmitted or appealed and the details are recorded to prevent the same issue from happening again.
Aging balances are monitored by payer through the portal status calls are made claims are. Records are requested when needed.
Iowa Medicaid enrollment, individual provider credentialing, Medicare revalidation and CAQH updates are managed for all existing providers.
Reports are generated on denial rate, days in A/R, collections by payer and collections, by service line.
Wellmark and Medicaid plan payments are checked against contract rates and any underpayments are sent back for correction.
Cardiology coding
Our coders start with the physician’s documentation and match codes, modifiers and units to that note and to the payer’s modifications.
How a practice bills – worldwide or per component – relies on who owns the equipment, who does the study, and where the study is done. For each location, we document the arrangement and construct rules from it.
How a claim moves here
The sequence stays the same for every claim. What changes is the payer rule applied at each step.
Collect demographics, insurance cards and ordering provider pre-visit.
Verify active coverage, plan type, secondary payer and benefit restrictions.
Separate IA Health Link plans, fee-for-service Medicaid, Wellmark, BlueCard, Original Medicare and Medicare Advantage.
Verify that the provider and the service location are enrolled and contracted with that payer.
Does this member need clearance for this CPT at this site?
Compare the signed note to codes, modifiers, units and diagnosis support.
Run payer edits, correct problems and release claim.
Send the claim to the relevant payer ID and verify that it has been accepted.
Post payments, compare to contract rates and identify exceptions.
Work corrections, appeals, record requests, and aging claims by payor
Practice administrators need to be provided with rejection, payment and A/R numbers.
Pre-submission list
This list runs on every claim as a final quality check.
Cardiology subspecialties
Each service line has its own codes, documentation, and payer traps, thus we have to build up rules for each.
Common challenges
These are the points where claims for Iowa practices most often get stuck, and the points we build controls around.
Frequently Asked Questions
Cardialogy Billing handles coverage checks and approval support. Cardialogy Billing also reviews coding, enters charges, revises claims, submits them posts payments, deals with denials, follows up on accounts receivable, enrolls patients and reports data. Some practices decide to outsource the process. Others prefer to outsource the parts that are slow.
The majority of Iowa Medicaid members are enrolled in one of three managed care plans: Iowa Total Care, Molina Healthcare of Iowa or Wellpoint Iowa. We also serve members who're still in fee-, for-service Medicaid. Each plan has its site and restrictions so claims are routed by members, not practice.
Some do, and some do not. The answer depends on the payer, the product, the CPT code, and the site of service. We pull the current list from each payer before the service is scheduled and match the approval to the claim afterward.
Iowa has 82 designated critical access hospitals, and many cardiology groups see patients in them. Place of service, billing provider, and who bills the technical portion all depend on the arrangement. We get that arrangement from the practice and build separate rules for each site.
Blue plan members are handled through BlueCard, with Wellmark forwarding the claim to the home plan. Out-of-state Medicaid usually requires the practice to be enrolled with that state's program, so we check this before the visit rather than after a denial.
By who owns the equipment, who performs the study, who reads it, and where it takes place. Practices that read studies without owning the equipment bill with modifier 26. Practices that own the equipment and read in the office usually bill globally.
Medicare is billed first. We then confirm the crossover reached Iowa Medicaid or the member's managed care plan, and we follow up on any cost-sharing balance that was not paid.

Coverage checks, approvals, coding, submission, posting, denials, A/R, and enrollment are all reviewed by Cardialogy Billing as a single, integrated process. A quick analysis reveals which payers are at fault and where claims stall.
