Integrated systems shape the billing environment
Most cardiology care in Maine is delivered inside one of two large integrated systems MaineHealth and Northern Light Health rather than through fully independent single-specialty groups. MaineHealth's cardiovascular network runs clinics across Augusta, Lewiston, Rockport, Scarborough and Waterville under MaineHealth Medical Group, while Northern Light Health's cardiology program is anchored at Northern Light Eastern Maine Medical Center in Bangor, including structural heart and adult congenital heart programs.
For an independent or smaller cardiology group, that concentration matters at the billing level as much as at the referral level: credentialing, facility fee-splitting on shared imaging equipment and hospital-outreach coverage arrangements all have direct claims consequences that don't show up the same way in a state with a more fragmented hospital market.

When a cardiology group provides outreach coverage at a CAH, the facility may bill professional services under Method I, where the cardiologist bills Medicare directly, or Method II, where the CAH bills for both facility and professional components under a reassignment agreement depending on how that particular arrangement was structured.
Telehealth is part of the workload
Telehealth follow-up visits, expanded in recent years through MaineCare reimbursement policy, have partly closed the distance gap for stable patients, which means place-of-service and modifier accuracy on telehealth-billed cardiology visits has become a real, ongoing part of the coding workload rather than an edge case.
Specialized billing catches distinctions before denial
Cardiology billing is unusually layered even before geography enters the picture. A single patient encounter can generate an E/M code, a diagnostic test code, a technical-component and professional-component split and a modifier governing whether that test was performed the same day as a related procedure.
National Government Services, the Medicare Administrative Contractor for Maine under Jurisdiction K, maintains active Local Coverage Determinations for cardiovascular stress testing and stress echocardiography and for transthoracic echocardiography.
